25 year old man with presumed cyclical vomiting, presented with post prandial chest pain and vomiting. His blood gases showed the following:

PH = 7.55

pCO2 = 23 mmHg

pO2 = 25 mmHg

HCO3 = 20 mmol/L

Hb = 192 g/L

Na = 142 mmol/L

K = 4.3 mmol/L

Cl = 104 mmol/L

Glucose = 9.0 mmol/L

Lactate = 6.8 mmol/L

Creatinine = 253 umol/L
